How to access our services
We offer a variety of services and there are many different ways to access them through rebates and referrals. We have provided some information below, however we also have a Care Coordinator that you can contact via Phone (08 6110 0570) or Email (care@perthdiabetescare.com.au) who can assist you in getting the right information and services for your health needs.

Fee Information
Our fees can vary depending on the service you may be attending therefore please contact our office on 6110 0570 for further information.
Medicare Rebates
If you are referred by your doctor you may be eligible for a Medicare rebate which can vary from $52.95- $85.00 depending on the type of referral and service being accessed. There may be a gap ranging from $5-$80 which can again vary depending on the type of service and whether you are a pension or health care card holder.

Private Health
You may be eligible for a rebate from your private health insurance provider for some services depending on your level of cover. Also our neighbour Friendlies East Victoria Park offer the 12 week health program which can be eligible for a rebate for HBF members and includes visits with our Dietitian's and Exercise Physiologist.

Workers Compensation and Motor Vehicle Accident Authority Claims
When booking your appointment, please let us know that you have an insurance claim. Where the insurer has accepted liability and pre-approval for the sessions has been accepted, we can bill the insurer directly. You will need to give us written confirmation that your insurer has agreed to pay for services requested.

Veterans Affairs
Eligible DVA card holders will be bulk billed. A referral from your GP is required
.
Payment Facilities
We have a HICAPS terminal onsite which allows us to process rebates immediately avoiding the need to submit a manual Medicare or Private Health claim after your visit.
All payments are due at the end of appointments and we accept cash, eftpos and credit card for payment.

Confirmation of Appointments
For your convenience we provide an automated SMS reminder before your appointment.

Cancellation Policy
If you need to alter or reschedule your appointment please provide at least 24 hours notice by calling our office on 6110 0570. This will allow us to give valuable appointment times to other patients.
Please be aware that a cancellation fee (appointment cost) is payable where less than 24 notice is given.
