Diabetes Education

At Perth Diabetes Care, our Credentialled Diabetes Educators focus on self-management education and have in-depth knowledge on all aspects of Diabetes. Our Credentialled Diabetes Educators are health professionals who have undergone further study and have considerable experience in their fields (Pharmacist Diabetes Educators, Nurse Diabetes Educators and Dietitian Diabetes Educators).

 

Having a multidiscipline team of Credentialled Diabetes Educators, Perth Diabetes can assist in making living with diabetes less stressful. The Perth Diabetes Care team can provide education on all types of diabetes such as:

  • Type 1 Diabetes

  • Type 2 Diabetes

  • LADA (Latent Autoimmune Diabetes of Adults)

  • MODY (Mature Onset Diabetes of the Young)

 

We offer an individualised approach, working with you to determine identified needs, goals and take into consideration your life experience and knowledge. Our Diabetes team can assist in all aspects from providing technology such as:

  • Latest Glucose Monitors

  • Continuous Glucose Monitoring

  • Flash Glucose Monitoring

  • Bolus Calculators

  • Insulin Pump

To providing links to other health professionals for particular focus such as:

Or it can be an appointment to further understand diabetes or diabetes medications, its effects, monitoring and recommendations. We will work closely with you and your healthcare team.

 

Our services are also open to your support network such as family members, friends, work colleagues, work places and carers.

How to access services at Perth Diabetes Care

There are many ways you can access our services:

  • Self Referral

  • GP Referral on a GP Management Plan which may allow you to access Medicare rebates if you are eligible

  • Endocrinologist Referral

Once we receive your referral, reception staff will contact you to make an appointment. Referrals are current for 12 months. You can also book an appointment by phone (08 6110 0570), email or online.

We have a wide range of consultation times and clinic locations available. Perth Diabetes Care has locations North of the River and South of the River, East Victoria Park is our headquarters.

Consults are available Monday to Friday (appointments are available after hours) and weekends – this includes Saturday and Sunday. Perth Diabetes Care can also offer Telehealth consults (video or phone consults).

Consult times can vary, we usually allocate a long consult (1 hour) for an initial appointment and a short consult (30 minutes) for a review appointment. Consult lengths will be determined on your discussions with your Credentialled Diabetes Educator.
