What is a Home Medicines Review (HMR)?

A Home Medicines Review (HMR) is where an Accredited Pharmacist upon referral from a client's GP will conduct a comprehensive clinical review on a patient's medicines in their home. â€‹

The service is a collaboration between the clients's GP, Pharmacist and other involved health professionals, it facilitates collaborative working relationships between care groups enabling between understanding and knowledge of client care.

What are the benefits of HMR?

By participating in a HMR you will see an increase in your knowledge, a holistic outlook, improvement in your medication, a growth in seeking healthier behaviour, strengthened self-management and interest from your family in friends who may also wish to seek out a HMR.

 

How to access a HMR?

You need to visit your GP and ask him/her for a HMR referral.

Your GP will complete the referral with you in the room and send it to your pharmacist.

Once receiving and assessing your HMR form, your pharmacist will call you and make a suitable time to come visit you in your home.
